Revise a Sponsor Information Sheet
Sponsors revise an approved Food Distribution Program sponsor information sheet as necessary throughout the program year. The CNPweb submits the revised version for approval and keeps track of all form revisions.
To revise an approved sponsor information sheet:
Change the program year to the upcoming program year (if necessary).- Click Program Year next to the program and your recipient agency name.
The CNPweb displays the Program Year Selection page.
- Click the program year.
The CNPweb changes the program year and updates the page display.
Display the Sponsor Summary- Applications tab.- Click the Summary menu.
- Click Sponsor Summary.
The CNPweb displays the Sponsor Summary - Applications tab.
- Locate the sponsor information sheet.
- Click Revise.
The CNPweb displays the Sponsor Information Sheet.
- Make all appropriate changes to the sponsor information.
- Check the certification checkbox.
- Click Submit.
The CNPweb checks the sponsor information sheet for input errors.
- If the CNPweb finds errors, it displays the Sponsor Information Sheet- Post Confirmation page with the error detection message. You must correct all errors and resubmit the form. For more information, see Correct Sponsor Information Sheet Input Errors
- If the CNPweb finds no errors, it displays the Sponsor Information Sheet - Post Confirmation page with the approved status message.
Notes:
- If the Revise option is not available, you cannot revise this information sheet. For more information, see Enrollment Form Tasks.
- The CNPweb updates the revision number at the top of the Sponsor Information Sheet.
- The CNPweb uses yellow highlighting to show changes on the form.
- If you do not check the certification checkbox, and the information sheet contains no errors, the CNPweb assigns the Pending Submission status.
- If you check the certification checkbox and the information sheet contains no errors, the CNPweb assigns the Pending Approval status.
- If you changed the program year to revise an enrollment form for the upcoming program year, change the program year back to the current program year.
- Your state agency can turn off the revise enrollment forms feature for the current program year.
