Enrollment Forms
Before the start of the program year, each sponsor must submit Food Distribution Program enrollment forms. The CNPweb allows sponsors to easily create and submit the appropriate forms because it identifies the specific forms each organization must submit.
- Sponsor Information Sheet. An application form that describes the organization and its participation in the Food Distribution Program. Every organization must submit one sponsor information sheet for each program year.
- Delivery Location Information Sheet. The CNPweb identifies the delivery locations operated by each organization that participates in the Food Distribution Program for the program year. Every organization must submit one delivery location information sheet for each delivery location for each program year.
Notes:
- Unlike the other CNPweb programs, the Food Distribution Program does not use packets to collect and submit applications at the start of the program year. You create and approve applications individually for this program.
- For renewing sponsors, the enrollment forms contain information rolled over from the previous year.
- If your organization shares a delivery location with another sponsor, only the sponsor that owns the delivery location must submit a delivery location information sheet.
- The CNPweb uses yellow highlighting to show the changes made from the previous enrollment form version.
