At the start of the program year, your organization must submit a sponsor information sheet as part of the Food Distribution Program enrollment process. This form describes your organization's participation in the FDP.
Statuses
The CNPweb assigns a status to each sponsor information sheet version and displays the status of all enrollment forms on the Sponsor Summary - Applications tab. The status identifies the location of the sponsor information sheet in the creation and approval process.
Revision History
Sponsors and state agency users can revise approved sponsor information sheets as necessary throughout the program year. When your organization submits a revision, the state agency reviews and approves it. The CNPweb keeps a copy of each approved version of the sponsor information sheet as part of the form's revision history.
Renewals and Rollover
The first year that your organization participates, you create a new sponsor information sheet from a blank form. In following years, the state agency rolls over some of the information from the previous year's information sheet before the start of the enrollment period. Your organization sees this rolled-over information in the new program year's information sheet. You edit the rolled-over information (if necessary), add the remaining information, and submit the form for the upcoming program year.
