Packet Online Forms

Your packet consists of the following online forms:

Sponsor Application - An application form that describes your organization and its School Nutrition Program participation. Every organization must submit one sponsor application for each program year.
Site Application - The CNPweb identifies the sites operated by your organization that participate in the SNP. Your organization must submit one site application for each site you operate that participates in the SNP.

Notes:

Your packet contains site applications only if your organization operates sites.
You must contact the state agency if your site application list is incorrect.
If your organization submitted an application packet for the School Nutrition program last year, the CNPweb rolls over some of the information to this year's applications. You can change the rolled over information if necessary.
